Right to Good Faith Estimate

Statement

If you’re uninsured or self-pay, you have the right to receive a “Good Faith Estimate” explaining how much your health care will cost.

 

Beginning January 1, 2022, if you’re uninsured or self-pay (don’t have your claims submitted to your health plan), you have a right to receive a Good Faith Estimate for the total expected cost of any health care items or services scheduled, or upon request, before those items or services are provided.

 

A “Good Faith Estimate” is an estimate of the total cost of the items and services that are reasonably expected for your health care needs. It should include expected charges for the primary item or service you’re getting, and costs for any related items or services that are provided as part of the same scheduled experience (like medical tests, equipment, prescription drugs, and hospital fees).

 

  • If you schedule a health care item or service with Empass Healthcare, Inc. (Empass) at least 3 business days in advance, Empass must provide you with a Good Faith Estimate in writing within 1 business day after scheduling.
  • If you schedule a health care item or service with Empass at least 10 business days in advance, Empass must provide you with a Good Faith Estimate in writing within 3 business days after scheduling.
  • If you ask Empass for a Good Faith Estimate (before you schedule an item or service with Empass), Empass must provide you with a Good Faith Estimate in writing within 3 business days after your request.
  • If your services scheduled with Empass include items or care to be provided by another provider or facility, you can also ask those providers or facilities for a separate Good Faith Estimate.

 

Make sure you save a copy of your Good Faith Estimate and the bill you receive from Empass.

 

If you receive a bill that is at least $400 above the Good Faith Estimate provided by Empass, federal law allows you to dispute the bill.
